Community Innovations Team

Community Innovations is a joint partnership initiative between Hampshire County Council, Hampshire Community Health Care, and the Council for Voluntary Services (in our area it is Basingstoke Voluntary Services).
As people get older, many things they've always taken for granted can start to become more difficult. Getting out and about, getting jobs done and getting by can be increasingly challenging. For many older people having a little bit of support to get those every day things done can help them stay independent and remain an active member of the community in which they live.
| This unique service aims to support older people who are not currently in crisis, but beginning to experience difficulties.

You can contact the team yourself, so can your GP, or any of your family, friends or someone close by telephoning 01256 362057 or you can contact Adult Services on 0845 603 5630 Hampshire County Council's website has more information about Community Innovations Teams across Hampshire One of the team will come and see you will talk things through to see if they can help by carrying out an assessment of what you might want and need, and develop an action plan with you. The Occupational Therapist and or Social Worker will provide specialist advice and support where and when you need it. You may be offered the assistance of a Community Support Worker to help you with some or your entire action plan. A newsletter has been produced by Hampshire County Council Community Development Workers have a good knowledge of what is available to you in your local area. If you and one of the team identify an activity or service that is not available the Community Development Worker may be able to support the development of a new activity or group/service. If you are a voluntary/community organisation providing a servie to older people please contact Pam on 01256 423828 so we can advertise your services Once the Community Development Team has searched an area to see what services are available they produce a Signpost Leaflet.
